Primary Care Services to all ages at 3 sites. If specialized medical
services are needed, our Health Support Services Representatives
will help make referrals to these physicians for you.

Dental Services

hygienists will work with you and your family members to make good
oral hygiene possible. Cases requiring extra care will be referred
to the proper specialist by our Health Support Services
Representatives.

Across the Smiles
ur mobile dental van provides dental care to children in many
Northeastern Connecticut communities. You will see us at local
schools, day care centers, Head Starts, community centers and
libraries.

Case Management Services
ase Management Services provide help with health insurance
questions, referrals to specialists and for tests. Health Support
Services Representatives will also help determine what programs may
provide help with paying for your health care or obtaining
medications.

HUSKY Plan
he HUSKY Plan is designed to help all children who don’t have
health insurance or who don’t have enough insurance and is funded by
the state and federal governments. It will cover children from birth
to age 19. It will help provide love cost or free medical, dental,
or behavioral health services and medications. Generations’
specially trained Health Support Services Representatives can help
with determining your eligibility for HUSKY coverage.

Nurturing Families Network
he
Nurturing Families Network is a no-cost, voluntary program that
provides information, guidance and assistance to first-time parents.
The mission of the program is to work in partnership with first-time
parents facing the challenges of parenthood by enhancing their
strengths, providing education and creating community connections.

Health Care to the
Homeless (HCH)
enerations
Family Health Center receives Health Care to the Homeless (HCH)
funding agency wide. In addition, each site uses various
resources to provide quality healthcare to homeless
individuals. Some examples of the type of resources utilized
are as follows: local hospitals’ mobile van, university
medical students, emergency shelters, soup kitchens, and
faith-based organizations. Educational activities, flu
vaccine clinics and blood pressure screenings are some of the
ways we are able to provide the much needed quality health
care to the homeless population. The success of Generations
programs are contingent upon the team approach with various
social services agencies. Our mission involves commitment to
provide quality, compassionate and professional health care
that is affordable, easily accessible and without
discrimination to all members of the communities we serve.
Through this program, we had 3900 visits by homeless patients
in the 2003 calendar year. |

Healthy Start worker will help find healthcare, free health
insurance and community resources to help keep you and your new
family in good health. |
